NEW PRACTICE MEMBERS

IMPORTANT STEPS BEFORE YOUR FIRST APPOINTMENT

Please plan 2.5 hrs total for your first appointment. The office visit will be up to 2 hours but please also plan for check-in and check-out.  If your appointment is virtual, this may be less of an issue, but still plan to have some buffer on either end.

 

1. Fill out your intake form, which should have arrived in your email already.
Please make sure to fill all fields. This intake is critical to a thorough Functional intake.

2. Get copies of labs and testing (urinary or salivary testing for eg) from the last 2-5 yrs.
We do not need your “records” or visit notes, ONLY labs and testing. Please contact your health care provider(s) directly to get copies of these.

3. Submit intake form and labs/testing to us at least 4 work days before your appointment.
This will ensure your first visit is as efficient as possible, and our providers have enough time to actually review your case before the meeting. To submit these documents, you can:
